The gist

Parnassus MICU runs two teams — resident-staffed Blue and APP-staffed Gold. Days: 3–4 residents + 1 medicine intern + CCM fellow + attending. Nights: 1 resident ± 1 intern. Directors: Kathleen Liu (MICU), Wendy Smith (rotation).

Your night week
  • Shift 5pm–6:30am — arrive ~5pm for sign-out; night rounds start 9:30pm.
  • Admit all new MICU Blue patients until 6am, split 1:1 with the night resident; build the morning distribution on the board; sign out at 6am.
Your day week
  • Day team takes sign-in from the night resident by 6am. Holdover presentations are brief sign-outs — full plan gets discussed on rounds.
  • Resident long shift 6am–7pm holds the admitting pager until 5pm; short shift 6am–5pm.
  • Sign out before 5pm to the long-call resident; after 5pm to the night resident. Always check with the fellow before leaving.
Tips
  • The MICU fellow is notified of every admission; the attending is in-house and always on Voalte.
  • ICU transfers: the ICU team is primary immediately, even before the patient arrives — bedside sign-out within 30 min of the MOD page.
  • Notes: ICU writes the H&P on transfer in; on transfer out, ICU writes the progress/transfer note and the floor writes the H&P. Call room in 9ICU is reserved for the MICU resident.

Day 1
  • Interns arrive 6:30am to pre-round; the night resident’s holdover email with patient assignments lands at 6am.
  • Call rooms: MICU long call H3028, night float H3029 (badge access).
Typical day
  • 6am holdover email → 6:30 interns → 7:00 residents → 8:15 code huddle (H32) → 8:30 rounds → 12 noon conference → 5pm non-long-call teams sign out to the long-call intern → 8pm sign-out to the night resident (“Manta Ray”) → 9:15pm code huddle.
  • Caps: team 8, admitting 5; long-call resident is code leader until Manta Ray arrives at 8pm.
Tips
  • Confirm every admission with the charge RN (x60529) and use the Critical Care Orderset.
  • ICU triage note for every consult; staff with the fellow (x69412) or attending within 90 min.
  • Transfers to wards: warm bedside handoff with your senior — only to holdover or long-call teams, never to swing/overnight. Transfer summary + discontinue ICU-only orders.
  • Codes get a significant-event note (“.codeblue” dot phrase); consent notes go in the physical chart; death note = discharge summary within 24h.

  • Sign-in from the night team (“Wombat”) before 7am; whole team present at 7am on call days.

Typical day
  • Interns start 6:30am. Long call 6:30am–9pm · post / post-post until 5pm · pre-call until 4pm.
  • Both daily conferences in M989 are expected.
Call cycle (q4)
  • Long: holdovers + admissions 7am–6pm — max 7 patients (6 fresh), decay cap 2 after 5pm; 7–9pm whole-team sign-out to Wombat.
  • Post: nothing new · Post-post: up to 2–3 holdovers (intern may be off/clinic/IHD) · Pre: no admits, resident or intern may be off.
  • Bounce-backs (<7 days) return to the original team any day; formal sign-out 8:30–9am.
Tips
  • Call the ED back within 15 min of the MOD page, eyes on the patient within 30 min, orders (Medicine Rapid Bed Request + Core IP Adult Admission) within the hour — goal is 60 min page-to-admission.

  • Sign in from the overnight hospitalist (“Crane”) by 7am.
  • Comms run on Microsoft Teams: “Cardiology Floor” (discharge planning — respond by 9am) and “CICU” (clinical).
Typical day
  • Teaching every weekday 8:30am in M1179.
  • Sign-out: to “Raven” (swing, 2–8pm) before 8pm, to Crane after; earliest recommended 3pm.
Call cycle
  • Long: admit 7am–7pm, team cap 10 (6 new), decay cap 2 after 5pm; interns 6:30am–9pm.
  • Post: no admits (to 5pm) · Short: up to 4 holdovers incl. bounce-backs (to 5pm) · Pre: to 4pm.
  • Crane takes all overnight admissions and cross-cover; “I-CON” med-consult attending (7am–7pm) for complex non-cardiac issues. Bounce-back = readmit or CICU transfer-out <7 days, returns to you.
Tips
  • Change first-call for holdovers right after sign-out.
  • Place “discharge referral to cardiology” orders ≥ 48h before discharge — one order per referral (EP + gen cards = 2); appointments then auto-appear on the AVS. Escalate discharge barriers to Chris Barnett.

Day 1
  • Arrive 6:45–7:10am; take sign-out from the night intern (“FIREFLY”) by 7:10.
  • Team rooms: T1 1A-63 · T2 1A-94 · T3 1A-13B/C · T4 1A-68 · T5 1A-65 (door codes in the guide).
  • Schedule + days off live in the VA Box schedule, not QGenda. Sign-out doc: bit.ly/vaw-signout (open in Edge, logged into VA email).
Typical day
  • 7:15 holdover sign-outs → 8am code huddle outside 1A-62 → 8–8:50 morning report → 9–12 bedside rounds + MDR (Teams, bit.ly/VAW-MDR; slots 10:30–11:45) → 12–1 noon conference → afternoon tasks.
Call cycle
  • Long: 7am–9pm, admit cap 7 (decay 1 after 5pm); MOD phone forwards to your senior at noon; sign out to FIREFLY from 8pm, out before 9.
  • Then Post → Tiny (no new) → Short → Pre (7am–4pm, resident–intern day).
  • Non-call days: sign out to “DRAGONFLY” at 3pm (2pm weekends) once the checklist is done. Re-admits <7 days come back to the discharging team.
Tips
  • CPRS has no push alerts — call the RN for any urgent order (antibiotics, anticoagulation, AEDs). Vocera x27901.
  • Labs are drawn 0600/1300/2000 as “Lab Collect”; anything else is Ward Collect + call the RN (add-ons x22260).
  • Procedures always need a fellow/attending in the room — page FHS to supervise paras/LPs (pulm does thoras); order kits via RN text order >30 min ahead.
  • LST note within 72h of admission, and DNR needs an order, not just the note. Direct admits/transfers → page FHS. End notes with “I have discussed the case with my attending, Dr. ___…”.

LTU = Liver Transplant Unit (Moffitt-Long): primary management of hepatology and liver-transplant patients. Team = senior resident + 2 medicine interns + 1–2 surgery interns, backed by GI / transplant-hepatology / transplant-surgery fellows. (The manual describes the senior’s role — you’d be one of the two medicine interns.)

Typical day
  • 7–8:45am pre-round / problem list → 8:45–9:45 rounds (Zoom) → 9:45–10 MDR → 10am–finish ICU + walking rounds.
  • Fri 7:30–8:30am transplant-surgery M&M · Tue/Thu 4:30pm HCC tumor board (as needed) · Wed midday transplant selection committee.

Call cycle
  • Long: 7am–9pm; team cap 10, admitting cap 6; admit 7am–7pm (decay 1 pt 6–7pm, triage-only 7–8pm). Admits after 5pm are granted, then staffed next morning.
